If you watch a lot of CSB videos you can do CBS bingo on any of them:
- people were not trained
- no written procedures
- no formal risk assessment
- lack of regular maintenance
- no safety assessment of particular products used in process
- no adequate monitoring system for fluid levels / clear interface
- no clear labeling of chemical intake ports
- and so on
The bhopal incident was the reason why the CBS was formed, and although it never got that bad, these CBS videos show how a lack of regulations in America has caused so many preventable incidents, it’s sad.
